After a nurse found you stealing medical supplies from the pharmacy, the hospital fired you. Now that you've lost your income from both your fencing business and your medical practice, you're left with no way of paying back the loan sharks you owe. So with the prospects of prosecution by the hospital and the threatening of the loan sharks, you flee your glitterworld homeplace on a fast ship to the rim.
Your stunt might have evaded the authorities but the crime ring the loan sharks work for have strong ties with the rimworld pirates. They find you and demand you pay up or die. You promise them that you have a way to pay them back you just need more time. You'll open an organ harvesting business and make payments every quadrum. Suprisingly, they take you up on your offer, but if you miss a payment again they'll come for you. Luckily, many hopeful refugees and fugitives flee to this rimworld for asylum. Trusting your charming smile and great bedside manner, they request your medical attention.

I recommend adding mods that make surgery more exciting like Expanded Prosthetics & Organ Engineering, FashionRIMsta (for the scrubs and surgical mask), and Harvest Everything! I would also recommend Not Enough NLW's as a way to detain any escapees and such.
